Ontario SABS sections 15 and 16 address reasonable and necessary transportation tied to treatment, counselling, training, assessments, and examinations. Authorization remains the insurer’s decision. An OCF-18 sets out a treatment and assessment plan. An OCF-21 is submitted through HCAI by licensed health service providers, not by transportation suppliers. Our rehab transportation services support scheduled outpatient visits and planned program arrivals for independently mobile riders. Toronto Rehabilitation Institute’s University Centre is at 550 University Avenue. West Park Healthcare Centre is at 170 Emmett Avenue. St. John’s Rehab is at 285 Cummer Avenue. The rider walks to and from the vehicle on their own. The chauffeur handles the door and ordinary bags.

These centres run different outpatient programs, so one schedule should not be imposed on every rider. West Park lists physiotherapy, occupational therapy, speech language pathology, and return to work programs. St. John’s lists several progression stage outpatient programs and requires referrals for most services. Toronto Rehab operates across several Toronto locations. We book the address shown on the appointment notice and do not assume the campus from the program name.

The Lexus ES sedan seats up to 3 and suits most solo riders with one or two companions. The Executive SUV seats up to 6 and offers more cabin and bag space. The Cadillac Escalade also seats up to 6.
